Ella Cox joined the practice in November 2022. She enjoys all areas of general practice but has a special interest in respiratory problems and runs a weekly asthma/COPD clinic.  

Usual working days: Monday, Tuesday, Wednesday, Thursday, Friday

Sangeeta is an experienced Senior Clinical Pharmacist prescriber and carries out medication reviews.

 

She has a special interest in cardiology and runs clinics to review high blood pressure and cholesterol management.

 

Her role also includes the delivery of clinical supervision to the junior clinical pharmacists across practices in East Waverley PCN.

Care Coordinators
Our care coordinators work closely with our doctors and nurses in helping to support people with complex needs in the community, particularly older patients and those who have recently been in hospital.

As Care Coordinators, our aim is to help people improve their quality of life. We work closely with GP’s and clinical staff to proactively identify and work with our patients, including the frail and vulnerable and those living with long-term conditions, to coordinate and help navigate care and support services. This is achieved by reviewing a person’s needs, and once identified, helping them to access support and services that can address these needs, while acting as a central point of contact for them.

 

We are able to refer to Social Prescribing, who provide a variety of services, including Health & Wellbeing and Financial Help - Occupational Health, for mobility aids – Adult Social Care, for care assessments – Continence Service, for continence assessments and incontinence supplies & a number of other professional services where appropriate, and based on what is important to the individual.

We also provide support for the carers and families of people living with complex needs.
